Your question is a bit vague, but I'll do my best...if you're not familiar with trans physiology in general, I'll provide a short overview here.

Lu Guang in this story is a transgender man, which means he was assigned female at birth and is now living authentically as a man. He is socially transitioned, which means he interacts with the outside world as a man, using male pronouns, etc. Many (but not all) transgender people experience gender dysphoria, and pursue gender affirming medical care to treat it. In this fic, Lu Guang is receiving that care. He is on T (testosterone), which can lead to a variety of physiological changes (as described in the link). He has not had bottom surgery, but he has had top surgery, using the keyhole method. Considering that he has had surgeries, and considering some of the other physical descriptions that I give throughout the fic, it is implied that he has been transitioning for several years by this point and that he likely started in his teenage years.

There are way more gender affirming treatments than the ones listed above, but these are the ones relevant to this specific fic.

Not all transgender people receive all of, or even any of, the gender affirming treatments available. Some choose not to for a variety of reasons, or start and stop as their own personal circumstances change. A trans person is still trans, even if they are not receiving or pursuing gender affirming care.

Many trans people refer to their genitalia with terminology appropriate to their gender. So for example, though Lu Guang has not had bottom surgery, he refers to it as a dick, as is reflected in this story. Others may call it a t-dick. Others may call it a clit. What each person calls it is of course a matter of personal choice, and there is no wrong answer. Trans women have similar terminology considerations for their own genitalia. In general, the most polite way for another person to refer to a trans person's junk is to not refer to it at all, just like one would exercise discretion for a cis person.

Dysphoria is a very, very serious issue. I hope that the distress it can cause is well illustrated in the story I wrote, but gender dysphoria can lead to increased depression and anxiety, increased social withdrawal and isolation, and markedly increased risk (up to 73% higher) of suicide (Source). This is why access to gender affirming/transitioning care is so crucial, and why I think it's so important to show trans people living authentically in fiction.

I happen to be a healthcare professional with a focus on transgender health, as well as a trans person. While I am comfortable explaining these things to you, I would also like to gently express that asking these questions of a trans person you don't know well is usually considered rude.
